Key Responsibilities:
- Review and correctly code medical records for Denial, Ancillary, and Emergency Department (ED) facility encounters using ICD-10-CM, CPT and HCPCS code sets and coding guidelines.
- After reviewing denied claims, determine what issues caused the denial and relate findings to the coding.
- Complete changes to coding and submit the claim per payer / company policy.
- Please enter the diagnosis and procedure codes as necessary.
- Check all the documentation to ensure all coding is correct and meets the standards for documentation of medical necessity.
- Coordinate with doctors, clinical team, billing and denial team, etc., to address coding problems.
- Carry out quality control on coded records and take corrective action as necessary.
- Keep up to date on coding requirements and those of the different payers and reimbursement methods as well as standards of practice.
- Attach documentation to explain services provided to patient.
- Reduce denials for incorrect coding by reviewing and giving constructive feedback on issues to do with denials.
Eligibility Criteria:
- Skill to assess and understand physician notes and clinical records, and review medical records.
- Awareness of insurance payor specific guidelines, reimbursement strategies and regulatory compliance.
- Good communication skills to interact with all.
- Ability to work with minimal supervision with required productivity and quality.
